SUPPORT TO INDIVIDUALS AND THEIR FAMILIES IN THEIR OWN HOME
If you need support to stay in your own home or your family home, we can help you. Our staff have experience of supporting people who have learning disabilities, physical disabilities, mental health problems, dementia, physical health problems, communication difficulties, behavioural difficulties, sensory impairments.
You can contact us direct, or ask a social work or health professional to contact us on your behalf. We will then offer you:
- a person centred planning process, to help you identify what you want out of life and what kind of support you need to help you get it
- a financial assessment to help you identify what your support might cost and how these costs might be met
- a range of supports (with an agreed support plan), worked out in partnership with you, your family and any social work or health professionals involved in your care. This could include assistance with aspects of everyday living, learning new skills, meeting people, doing interesting things in the community
- the opportunity to be involved, with your family if you choose, in the selection and training of the staff who will support you
- the opportunity to be involved, with your family if you choose, in monitoring and evaluating the support you get and making sure the results feed into planning your future
In addition to direct support staff, we have a small team whose job it is to help identify or create opportunities for you to develop your interests and meet new people. They will work with the person centred planning process and then with your support staff to help you realise your personal goals.
If you need help to find somewhere to live, we will help you negotiate with housing associations, if you want to rent, or if you want to buy, we will help you negotiate with banks and building societies. If some of your support costs can be met from Supported People funding, we will help you complete the appropriate applications.
