| You may be someone who is supported by ELCAP; a relative, carer, visitor, friend or a staff member who is dissatisfied with the service you have received or witnessed. |

| ELCAP has a procedure to help you make a complaint and this page explains how to go about it. |

| If you have a complaint or suggestion: |

Tell a member of staff in the first instance. |
|
|
- |
They will try to resolve things for you at once and will give you a suggestions and complaints form/leaflet to complete or take away along with a s.a.e. You can find all the information in the leaflet on this section of the web site. You can submit your suggestion or complaint online. |

- |
You do not have to complete the leaflet/form or online form to have your suggestion or complaint followed up by staff. |

| We will write to you within 7 days telling you how we will follow up your suggestion or complaint. |

| Within 4 weeks you will receive a letter informing you of: |

the outcome of your complaint and how any issues raised have been resolved |

how your suggestion will be taken forward |
